The forms listed on this page are required as a part of the application process.​ All forms listed below must be sent to Courage Schools directly from the school/evaluator via mail or email.

Release of Educational Records (required for all applicants)

(Confidential Teacher Recommendation forms are handled through the online application process)

Tuition
& Fees

Tuition includes books, curriculum materials, classroom supplies, and field trips. Speech and language support and occupational therapy support are built into the school day and are a part of tuition.
